One pixel in the config does = one km. One tile in the game is 16 meters. However keep in mind that even though this will get your geographical scale down, there are other scales in the game that are messed up. You will need a commute patch or else your sims won't even go more than across one of your large cities (3 - 4 km). Then, the population densities of the game mean that even a city full of houses contains a lot more people than it would in real life.
