Thanks, Michelle. Maybe the key paragraph is number 9:

9. It is often the case that an agent, a manager, a
studio, a production company or a broadcast
entity will facilitate the entry by filling out the
form, but the individual who is listed as the
"eligible entrant" is considered by the academy
to be the person who has made the entry.

My head kinda hurt after reading all those rules.
