I am a tech from Texas who is currently working in Ontario. They do use the term "pooched" to mean that something does not work. I have been enquiring amongst the locals as to the origin of the term. It can also mean that some has passed out or been fired. It can also be used to describe the fact that an action has failed.
I am guessing, but it may be derived from "scewed the pooch" in some way.
As for the lady from Ontario: I don't know how you could not have ever heard this term. I have heard it countless times in 3 cities in Ontario in many different contexts.
