I think that the most obvious reference is to the fall of the woman on the bridge. This is the start of the laughter and this begins his awareness of his own flaws and hypocrisy. Of course, after the woman falls from the bridge he falls into introspection and his whole life begins to fall apart, so it could defenitely refer to the fall that his whole life takes as he turns into a judge penitent. Overall though, I think that he is referring to the fall of the woman on the bridge.
