Have you tried Clicker 5?

You could read with the child at the same time.
Check that the child understands the story line by asking a few simple questions and then get them to ask you a few questions which is fun!- confidence building.

Share the paragraphs to read - child reads one and then you and so on.

Words that they are not sure about make a word bank for spelling and meaning. They could also draw the item e.g. Television.

You could ask the child to turn the book or just part of it into a storyboard - great for sequencing and checking the level of understanding. - a visual and creative way to learn. Also re-enforces what they have already read.

Listen to audio tapes - great for concentration.
Maybe have an audio tape and book at the same time.

Try reading in different environments - outside, in a quiet room or library etc
