 Ok then here are the questions, feel free to answer please.

1. What is your psych disorder (try to focus on only one and not your ED)?
2. How has it affected your life?
3. Does it keep you from enjoying life?
4. Are you in treatment(i.e. meds, therapy)? If so, please desribe.
5. How did your family react to your disorder?
