Compared to the multiplicity of body piercings that people get up to nowadays, this is comparatively minor. Mind you, at Jim's Forum, we instead advise people (OK, guys) to spend the time to stretch it instead.

Why do we say that? Well, there is some slight risk in amateur surgery, so if you insist on having it tied, it is preferable to have a doctor do it who is experienced and has the necessary materials ready to hand, and will (hopefully) dissuade you from such foolishness as putting "antibiotic" on afterward.

The proper doctor for this is generally a "GP" - General Practitioner rather than a "urologist" as the latter are often too eager to attempt to "sell" a circumcision. Clearly, this is a very simple and straightforward procedure performed only in the consulting room, and even anaesthetic is hardly necessary when the operator is experienced and has the correct materials ("3/0" nylon suture), though Emla� certainly is useful if you are squeamish.

The main reason for advising stretching rather than tying is that it is physiological - it emulates the natural process by which the fraenulum should have been caused to grow by regular retraction (including during dreaming) through childhood and puberty, and because it engenders a feeling of accomplishment of having "done it myself". Also, it does not leave spurious skin tags, for what that is worth.

Again, there is no structure of critical importance in the fraenulum. It may have a tiny artery, but the body will automatically provide alternative perfusion if this is interrupted, and the same for any veins. Any nerve it contains is absolutely trivial for you see, nerves radiate outward from the spine and approach midline structures from the sides, they have no need to travel along the fraenulum.

Why does it exist? The fraenulum probably has two functions, one is to pull the foreskin forward so that it automatically re-covers the glans as you lose your erection - the rationale for this is obvious. And people say it is "sensitive" but it is not particularly sensitive in itself - what it does, is to transmit motion from the foreskin to the urethral meatus (pee hole) which certainly is sensitive. You can sit and argue of course, whether such sensation is important or not.

I do note however, that much of this discussion of "Fraenulum Breve" is basically cosmetic; the demonstration of the glans being pulled downward hinges(!) on two details, lack of a full erection (where the glans is of course, much more readily pulled downward), and pulling the foreskin back more firmly than should be expected in enjoyable sexual activity, which is predicated on adequate lubrication so that friction is minimal - otherwise it can be expected to be uncomfortable for both participants.
