I have been going through the main site and think the wisest thing may be to place the stitch horizontal with the shaft. I got the idea from this http://www.male-initiation.net/library/drawings/fren_clip.gif Similarly, if I place the stitch in the same direction as the red line in the pic, and as close to the shaft as possible, I should avoid a loose flap of skin left on the glans. Of course, there will be more excess skin on the shaft side but shouldn't be a problem during erection and may actually benefit from having more skin to stretch backwards.
