Subject: Phimosis -when is a good time to circumcise?

My son is 3 and a half. His foreskin is so tight that I have never seen the head of the penis. When moving it it doesn't open even a little bit. He has had one really bad infection at the age of two. (swollen, red, very painful) He had to go on antibiotics. It was very traumatic and very painful, something I never want to experience again. We saw a urologist who prescribed steroid cream. After six weeks of use nothing changed so we discontinued it. Hoping things would get better naturally we haven't taken the circumcision route yet. Though just recently his penis started to get infected again. We put antibacterial cream and it cleared up in a day. (not nearly as bad as the first time, but scary as I did not want to got through what we went through the first time) My questions are: when is circumcision really needed? (what are the signs that the foreskin is not going to budge?) What age is best for the least amount of psychological repercussions? Is there an age that is better for avoiding any circumcision complications? If there is scar tissue from the first really bad infection will that make the chance of naturally "working" the foreskin to move back less?
