I was also wary about piercing/tying below the arteries/veins (you mention one in your report, but I think there's normally two!). I too was concerned about swelling and bleeding - and also what would happen to those vessels once they'd been cut through. I didn't need to go in below them; but I don't think I would have done anyway, not knowing what to expect.

You mention cutting out a ligature that isn't tight enough. Did you actually manage to do this? I seemed very tricky to me - the knot is just so small! When I had one that didn't seem to be working I just tied another ligature alongside it, using the existing hole.
