Have you seen the moon phase image at the top of the Gaia's Garden homepage? http://www.gaias-garden.co.uk/
If this is what you are looking for then click on the moon and it will take you to the website where I got the code from. Lots of useful astrology/planetary/lunar info to use.

Another good one is : http://www.calculatorcat.com/moon_phases/daily_moon_phases.phtml

You may find the 'Lunarlore & The Moon' links page interesting - visit the GG main Links page for the directory. (Failing that Google 'moon calendar for your site' or similar words).
