Subject: 350 B3 Accident

EUROCOPTER has been informed of the following incident:
A hydraulic power system failure appeared to occur on an AS 350 B3 aircraft during a flight. The
pilot was alerted by the "HYD" warning light and the "GONG". Upon these warning indications, the
pilot attempted to land in accordance with the Flight Manual "Emergency Procedures".
The pilot had great difficulty in controlling the yaw pedals, which resulted in a hard landing.
Investigation revealed that the "HYD TEST" push-button on the console was improperly left in the
engaged position. However, even after hydraulic failure training, the hydraulic test function must be
deactivated before landing.
The results of the investigation did not confirm any hydraulic failure, but rather pointed to a
hydraulic power cut-off due to unintentional actuation of the "HYD TEST" push-button. The pilot,
intending to switch on the landing light, might have pushed the hydraulic test button instead.

everyone's got a plan till they get hit in the mouth.
Funny thing is, you won't know this little secret until it happens to you. FACT IS; if you, your passengers, and the aircraft make it, then you have done your job well.

Learn from your mistakes and other peoples.
Now go poop in someone elses pot!
