Sino pregunta a ver donde está. Tienes que recoger los impresos que te den para cubrirlos (puedes preguntar aqui a ver si eres de diferente zona seguro que alguien sabe donde está).
De ahí, reúne todas las pruebas posibles, folletos, matricula.. puedes entregar las copias, no hace falta los originales (por si te hacen falta en otro momento)y toda la información que tengas, por ejemplo fotocopia de las portadas de los libros, fotocopia de la ley donde dice ..., lo que tengas..
por otra parte, para la redacción haz un borrador mejor contando todo lo que te afecte, incluye todo, y luego mira que te coja en la hoja que te dan puedes escribir por delante y por la parte posterior.
