borang boleh didapati di IPD berhampiran... sila bawa dokumen berkenaan (transkrip rasmi, ijazah, diploma dll).. borang hanya akan diberikan kpd mereka yg mpy tinggi&berat badan yg berkenaan (tak ingat le pulak)..setiap borang dikenakan RM seringgit... -- Forum Tanjung'93
