In the UK, the doctor and assistant are always female. Each patient is examined alone, and in their underwear. There is a visual check for hernia, testes, and penis condition. The foreskin is checked for retraction. Erections are variable. It's quite normal to see the beginning of one, and sometimes they become full very quickly. So obviously the genital inspection is kept as brief as possible and close to the end of the examination.
