In the US it varies from doctor to doctor. For school aged kids (5-11) normally it is down to just underpants. Some older doctor’s still have the younger kids take off their underwear at the beginning as well, but that is becoming less common as older doctors retire. At the very end all boys will be checked for hernias regardless of age. Boy’s whose foreskins are still attached will have them rolled back and looked at regardless of age as well. At 10 doctors begin to examine the testes and penis as well and also taking note of any signs of puberty (Hair, penis & testes growth). Many boys get embarrassed when the underwear is pulled down, especially the older boys with a female doctor, erections are common. Normally girls will have their underwear pulled down enough to take a quick look at her outside genitals at the end. Doctor’s begin taking a closer look when girls reach 10, again making note of signs of puberty (hair). Ages 12 and up both genders have a longer look with underwear off. But all this depends on what group the doctor is a member of.

AMA - American Medical Association
The AMA is the only one that does not suggest breast/genital examination for teens beyond checking puberty stage. What about France?
