Very nice your question!!I'm not sure if "linguistic choices speakers or writers made are(using a word instead of another, for example)" part of the co-text, because the author says that co-text or context refers to the "stream of prior and subsequent language in which a language segment or an exchange occurs" (p.12). I think we can say that it's inside and outside the text because there is one example, on the same page, which says that if a friend says "hello!" the person expects some oral response. So, inside the text, we can consider the proper oral language and outside the text is relate to the pragmatics: if a person is greeting another one, he/she expects an answer, this is logical thing, according to our context.
Let's wait Kátia's answer to confirm this.
