My definition of a puppy mill is somewhere that dogs are bred and sold purely for financial gain, regardless of the best interests of the breed being bred. Puppy millers can own 2 dogs or 200, it's the ethos that makes the difference, not the number of dogs. It usually entails bad management, no testing for any breed health or traits but great advertising. More effort goes into advertising to sell than breeding good dogs that sell themselves based on a credible and ethical reputation.
