The guiys I usually just have bend over and stick. The girls if they are thin I will have them lie flat and point their toes inward as it helps to relax the muscle. I usally will slap their cheek once and stick immediately. Their butt cheek is stinging from the bare hand slap and they normally do not feel the needle stick especially if it is a large needle.

I have to get b-vitamin shots periodically, and the doc has me pull down my pants/shorts and bend over the table. I have a friend who also gets these and he's always received them the same way, so this seems to be common.
