Warped Pressure plate: The pressure plate is #1 in the diagram, it is the the piece that contains the metal surface the opposes the flywheel; between the flywheel and the presure plate resides the clutch disk. The pressure plate and the flywheel are both friction surfaces that the clutch disk depends on to transfer power from the engine/flywheel to the transmission/pressure plate. A warped flywheel or pressure plate will decrease the amount of correct firction area and cause slippage.

The diaphram sping also #1 in the picture, connected to the pressure plate is the housing of the pressure plate that is responsible for applying enough pressure to the clutch disk so that it does not slip on the flywheel. The diaphram spring is directly pushed on by your throw out bearing #4.

Whenever a clutch is replaced without resurfacing the flywheel and replacing the pressure plate and diphram spring slippage is highly possible.
