Subject: i am trying find a poem for a friend but i only have some of the lines from it and no title

 the poem was read a funeral of a loved one, my friend is looking for it because her mother wants it read at her funeral. she believes the title is Hold Her Gently Jesus, because it is repeated through-out the poem. some of the other lines she remembers are-'she is very tired today'-'pain hidden with a smile'-'the cross she bares so patiently, becomes her crown today'
if someone could help me help her i would greatly appreciate it. thank you!
