It depends on age really. The older someone is and the longer they've been smoking the more obvious it is by the face, I would say.
I know a 50 year old woman who I never knew as a smoker, but who I thought in passing had almost definitely smoked at some point in her life before it ever came up in conversation, just from the lines around her mouth; sure enough it eventually came up that she smoked from her teens until 18 years ago, when she was in her early 30s.
But someone in their early 20s, for example? I'm not sure it shows at all yet for most people then. I guess shorter term things like stained teeth if they are negligent about dealing with that, but none of the really obvious long term signs.
