my impression was that the ex-girlfriend (Kim??) had broken up with Don because he chose to move home to be with his family during and after the time when his mother was dying. Kinda like she wanted to be the center of his whole world and was angry that he put somebody else (i.e. his dying mother) above her. Guess we probably won't ever really *know* what led to their breakup, but I'm predisposed to think well of Don because everything else we've seen about him in the show, like his concern for his father and brother, seems to indicate he's a nice guy.
BTW, it wasn't like Don's entire family didn't know about her. When Charlie showed a picture of Don and Kim to his Dad, his Dad recognized her right away and said something like "She was Don's girlfriend when he lived in New Mexico". It's been pretty apparent on the show that the Eppes famliy dynamics tilted strongly in favor of Don-and-Dad and Charlie-and-Mom, so Don probably shared a lot more with his father than he did with his brother and mother.
