The following is a translation of the takarafans.com survey. I used the ID number from my Cobalt Sentry, so I didn't get the additional Binaltech Questions. I'll stick a translation of the BT questions next time I complete the survey for a BT toy.

Go to Takarafans.com and scroll down and click the 'participate' button to begin. It's a big red button with white kanji in it, you can't miss it.

PAGE ONE
Enter your customer ID number. This will be the 9 character alphanumeric string on the slip of paper that came with your toy advertising the Takarafans.com site.

PAGE TWO
Privacy statement! Scroll all the way to the bottom and click on the left button to agree (right button to disagree).

PAGE THREE
Yay! You agreed!
The first field has four boxes. The top two require you to enter your name in Kanji (although technically you can enter in alphabetic Romaji if you don't have a Kanji name) - surname on the left, given name on the right.
The bottom two require you to enter your name in Furigana. For us Gaijin, that means Katakana. Again, surname-given name order.
The second field is for your address. The first set of boxes is for you to enter your 7-digit post code. Once you've done that you can click the Enter button next to it, and the form will automatically complete the next two lines, which are the pull-down Prefecture menu and a blank line to enter your city, town, village/district. The last line is for your ward number.
The third section is for your phone numbers. It allows you to enter 5 digits per box, allowing for a total of 15.
Remember that this survey is being read by someone in Japan, so you need to enter your phone number as if you were calling from overseas. Australians need to enter +61 for Australia, then followed by your state/territory number, minus the zero (e.g.: NSW is 2). So it'd be +61-2. However, this form does not allow special symbols like + and -, so just enter 612. Then enter your 8 digit phone number in the next two boxes - I entered four digits per box.
In the fourth section, enter your Email in both boxes - the second one is simply for verification.
In the fifth section, enter your mobile phone Email. Again, verify it on the next line.
In the sixth section, you click on your gender, left button is male, right is female.
The 7th section is for you to enter your date of birth. Use the pull down menu to select year, month, date.
The 8th section is for you to select your occupation. The options are: (first column, top to bottom) student, office worker/government employee/group staff, family-owned business, part-time/casual (second column, top to bottom) home-maker, not working, other (specify).
The 9th section asks for the number of people in your family. The options are (L-R) 1 person, 2 people, 3 people, 4 people, 5 people, 6 people or more.
The last section asks for which family members you live with. Options are (L-R) spouse, child(ren), parents, siblings, grandparents, other

Insert your JAN Code (last four digits of the toy's ISBN), then click on the Enter button. Voilà! The name of the Transformer appears on the page! Oooh!
The next section asks for the date that you purchased the toy - use the pull down menu to select year, month, date.
The third section asks you to select where you purcased the toy from. The options are (L-R, T-B): specialist toy store, discount store, 100-Yen shop, TRU, Itoyokado, Jusco, Daiei, electrical appliance store, convenience store, supermarket , internet shopping, gift, misc goods speciality store, other.
The next section asks who mostly uses the toy. Options are (T-B): yourself, male child (age), female child (age), other. At the bottom, you may enter your child's name and birthday (month, date).
The fifth section asks who purchased the toy. Options (L-R, T-B): yourself, father, mother, grandparent, sibling, relative, friend, other.
The sixth section asks how you came to know about the toy. Options (1st column): because I saw it in a shop, I saw it in a newspaper/magazine ad, I heard about it from a friend, I saw it in a mail magazine/flyer, (2nd column): I saw it in a TV show, I saw it in a pamphlet, I saw it at a convention, other, (3rd column): I saw it in a TV commercial, I saw a friend's, I saw it on a web site.
The last section asks you to select your reasons for buying this toy. Options (L-R, T-B): looks fun to play with, I like the character, I saw my friend's, I saw it in a store, I saw and played with it in a store, I saw it at a show/event/con, I saw it in an advertisement, I like Takara products, I'm attracted by it, other

PAGE FIVE
You'll see two buttons next to each option. You need to check boxes next to things that you have a particular interest in -- effectively, things that you spend your money on! Click the left box if you spend quite a bit on it for yourself, or the right box if it's more of a peripheral interest and you have relatively less interest in.
Options (first column): sports, trains, ecology, travel, pets & animals, internet, dining, mail order shopping, (2nd column): car, bicycle, models, board games, languages, movies/theatre, karaoke, (3rd column): airplane, motorcycles, music, computer/video games, comics & animé, beauty/diet/cosmetics, character goods (e.g. action figures).

PAGE SIX
Select a lucky draw prize course.
Options are:
A Course (family)
B Course (single)
Don't want a participation draw-prize

PAGE SEVEN
Here is the free prize that everyone gets for participating! Oooh! :p
The first button you click to get your desktop or mobile phone wallpaper, the second is to return to the survey home page.
