My daughter DC experienced similar problems, it really began to get her down as maths was the one area of school she was good at!!
I began by checking the department for education website for maths at her key stage. Having identified the language which was required and together created a table. Words such as sum, product etc were in the first volume, the corresponding mathematical symbol was in the second column and DC drew a picture of the function to remind herself in the third. It took quite a while to complete but it was well worth the effort and we laminated it to keep it clean.
