He once said he smokes so he can keep his hands busy, now that is just a lame excuse to avoid the truth that he's addicted to nicotine.

As far as his voice goes, it sounds fine to me, and always has. I think it adds character when its scratchy, but the down part of it is when they start touring, and he has to sing and keep singing, it wears him out, and it'd wear anyone's vocals out. He once was so voiceless, he couldn't even get the words out to apologize. Songs like Bad are killers on the throat, so maybe they tuned the album to his vocals. But Brian Eno and Daniel Lanois and The Edge all contribute alot of vocals to most of the songs on the album, and I wouldnt be surprised if you see The Edge singing more than ever on the new tour, either that or they invite the two producers on tour for a few shows.
