12 Winners will receive their photo on a month of the 2004 Miss Diva USA Wall Calendar!!! Winners will also receive a Gorgeous Crown, a Lovely Custom Banner with the month that they win ("Miss Diva January"), a banner pin and a $50 certificate to be used at the 2004 Miss Diva USA National Finals!!!These Calendars Make a Great Gift!!!

Entry Fee: $30 for the first photo
$10 for each additional photo
Photo can be color or black & white and should be no smaller than a 5x7 and no larger than an 8x10.
Please write the name, age and address on the back of each photo.
If you would like to have your photos returned, please include a self-addressed envelope with sufficient postage.
Please be sure to package photos carefully, we cannot be responsible for photos damaged during shipping!!
All entries must be received by November 15, 2003. Any photos received after November 15, 2003 will not be entered and will be returned to you.
Winners will be notified by November 30, 2003 and calendars will be mailed by December 15, 2003 in time for Christmas.

By participating in Miss Diva USA events/photo contest, I/We hereby agree to release and hold harmless Miss Diva USA, its directors, agents, representatives, and any other contestants from any rights or claims for damages or injuries which may occur while participating in any activity connected with Miss Diva USA. Further, I/We also grant Miss Diva USA the right to use any photographs, movies or video-tapes made during any activity connected with Miss Diva USA, and to use our name with any of the before mentioned in any promotional efforts on behalf of Miss Diva USA and group competitions of this and subsequent years thereafter. I/We understand that under no circumstances will refunds be given unless we cancel the said event. Miss Diva USA reserves the right to substitute prizes with those of equal or greater value without notice. Miss Diva USA reserves the right to cancel an event if minimum contestant numbers are not met. I /We understand that any infraction of these rules will result in disqualification. This constitutes the entire agreement of the parties and is legally binding.
