He may have ear mite, but only a vet will be able to tell you for sure. They will need to take a swap and look at it under a microscope. The mite will be visible. Treatment is fairly simple and straight forward. One of my rabbits recently had ear mite and I still don't know how. She is the one that stays inside with me nearly all day, and none of the others had any problems.

As for the vet visit, uncomfortable and unpleasant as it is, you may just have to hold him while the examination is done. Take a towel or something for him to sit on, so he's not sliding around. It will also make it easier for you to keep a firmer grip. You could also try wrapping him in the towel with just his head exposed. It will stop a lot of the struggling.

For the nails I have used fine sandpaper on the ramp leading up to the second floor of my buns condo. It does a great job of keeping the nails in good shape. If your bun doesn't have a condo, see if you can construct a ramp and platform to place his food dish on. This method saves a lot of work and stress.
