Indeed the annotation support is interactive with the user. If you want to manipulate manually it will require some looking at the internals.

TImageScrollBox has a protected data member, FDisplayEditor of type TAnnotationsEditor from the EnAnEdit unit. This is a protected member so obtaining access to it can be done with a trick like,

TMyScrollBox = class(TImageScrollBox);


Then you will be able to add annotations manually, and you will have access to the annotations using the TAnnotationsEditor.Items property.
