I am not sure what to suggest at this point. I would make sure you have the latest twain DLL's, from www.twain.org.

I would also check for the latest drivers from the scanner manufacturer.

The last resort good be use something like the exception dialog in the Jedi library, <a rel=nofollow target=_blank href="http://sourceforge.net/projects/jcl/">http://sourceforge.net/projects/jcl/</a> which can display the exact call stack when an exception occurs. Exceptional magic can also do this. I have a feeling this is happening in a DLL (either twain or the scanner driver) though.
