It appears you have the Apple "OEM" version of the TwinTurbo 128 (designated by the "A" after the card's model number). The drivers in the "tt128xxx" installers do not work with the OEM card (see later messages in this forum).

Use the 9600 Graphics Accelerator extension found at the link on this site's home page.
