The 1995 error is nothing compared to the goof that happened in the summer of 1994. David Perry filled in for Casey and actually said "I'm Casey Kasem." Nobody caught it until a PD at a small market station noticed it.
As Perry explained, the mistake happened partly because he was flying through his copy so quickly that he didn't notice. Nor did anyone else in the studio.
