I am a female doctor, I understand your concerns perfectly.
but as a doctor my feelings are never moved by patients actually I rarely even notice what the are wearing or how do they look as I am always focused on finding out a diagnosis so I am looking at them in a way to find what's wrong.
Another important thing is that if you have feelings towards a patient you usually do a really bad job as a doctor because compassion and friendly feelings clouds your medical judgment that's why its never recommended to treat family members.
as for the smile, its just a welcoming face to make your doctor visit less scary or less stressful
