The smoke roll is added to the TO HIT attempt, see
parenthetical reference in rule 24.6).

Yes, you are right. That is what "at the outset" is
supposed to mean. On the Advanced Sequence of Play on
the back of your SL rulebook, note that 2.1 is for the
smoke placed by engineers (which does not require a roll)
while smoke placement attempts per rule 41.3 would have
to be "at the outset" of 2.2. Point here is that you are
not permitted to fire at an enemy target smoke-free and
THEN place smoke to protect you from the counter-fire.
