I'm a lefty and try to place the cigarette close to the middle of my mouth. Some people exhale out the side of their mouth. If at a table where smoking might offend some, I raise my head and exhale.
I hold in my right hand a great deal. Again, at a table, I hold in the hand that will be outside the group. Driving? Often I hold in my right hand and steer mostly with my left.
Dangles are brief and only done when both hands are busy.
