The answer to your questions:

1) Brian died by drowning in a swimming pool.
2) As far as the way the Stones treated him. They have admitted to treating him badly at times, but Brian didn't make things any better either.
3) Yes, Anita Pallenberg did leave Brian for Keith Richards.
