There's Never Been A Better Time To Become A 'Dazzling Doll'. Awesome Awards, Beautiful Large Trophies, and Gorgeous Crowns!!
Attire: Pageant Dress-Glitzy or Non-Glitzy, Outfit Of Choice, & 2nd Outfit Of Choice(Optional)
10 Supreme's & 7 Overall's Will Be Awarded! Don't Miss The Fun!
Dazzling Dolls Of Kentucky
"July Gems"
AGE DIVISIONS
Girls: 0-11 Months, 12-23 months, 2 Years, 3 Years, 4-5 years,
6-8 years, 9-11 years, 12-14 Yrs, 15&up
Boys: Categories for boys will be announced after all entries have been received. Boys up to 6 years may enter.
In Each Age Division There Will Be:
King/Queen:
The winner in each age division will recieve a Nice Trophy, Crown, & Banner
Alternates:
There will be up to four alternates awarded in each age division.They will recieve a Trophy, Crown & nice gift.
Future Queens/Kings:
Everyone else in the age division will recieve a nice trophy, crown, & gift.
Photogenic:
Each age division will have a photogenic winner. Please bring your own photos with you the day of the pageant.
Photos must not be any smaller than 3 1/2 x 5" and no larger than 8 1/2 x 10".
DO NOT MAIL PHOTOS!!!!!! Please bring photos with the name and age division of the contestant on the back, they will not be accepted otherwise. Color or Black & White will be accepted. The photogenic winner in each age division will recieve a nice trophy.
Photogenic is included in the $100 Package. You can submit more photo's for $5.00 each or $15.00 for unlimited.
Optionals:
There will be one winner in each age division for the following:
Most Beautiful, Most Eye-Catching, Prettiest Eyes, Prettiest Smile, Most Photogentic, Prettiest Hair, Best Outfit Of Choice. The winners will recieve a nice trophy.
Elite Grand Supreme:
The One Contestant with the highest score of the day will receive:
A nice Crown, Large Trophy, Banner & Nice Gift.
2 Grand Supremes:
We will have one grand supreme in 0-3 Yrs, and One grand supreme in 4 Yrs & UP
This award goes to the contestant with the second highest score of the day. They will receive:
A nice Crown, Large Trophy, Banner, & Gift.
2 Mini Grand Supremes:
We will have one mini supreme in 0-3 Yrs, and One mini supreme in 4 Yrs & UP
This award goes to the contestant with the third highest score of the day. They will receive:
A nice Crown, Large Trophy, Banner and Gift.
Dazzling Dolls Supreme, Dazzling Star Supreme, Dazzling Princess Supreme, Angel Face Supreme, Rising Star Supreme:
If you win one of these supremes you will receive a Large Trophy, Nice Crown, Banner, & Gift
Overalls:
This will include: Overall Photogenic, Overall Prettiest Smile, Overall Prettiest Eyes, Overall Most Beautiful, Overall Best Outfit Of Choice, Overall Directors Choice & Overall Judges Choice!!!! Winners will recieve a Very Nice Trophy(or similiar). We Will be giving away some very unique awards for overalls!!
Information:
Contestants will be judged on facial beauty-(1-50pts) Overall appearance/stage presence-(1-25pts) Overall personality-(ages 0-4)(1-25pts) Overall Modeling-(5&up)-(1-25pts). The stage will be in a back T formation. Admission will be $6.00 for everyone exept contestants. Bad sportsmanship will NOT be tolorated. Any displays of this will result in disqulification. And you will be asked to leave with no refunds. Don't let this happen to you. Pageants should be fun for children and parents too. It is a way to have fun and meet new friends along the way. We want our pageants to be honorable and lots of fun for everyone. We reserve the right to lower awards if less than 15 contestants. No one will leave empty handed. The judges decisions are final. We will not be double crowning our contestants.

$45.00 Deposit REQUIRED By Deadline!
If you have problems with this please let us know, we will work with you.
You must pay the remaining balance before the pageant begins or you will not be eligible to compete.
All Deposits are Non-Refundable. If for any reason you are unable to attend the pageant we will hold your deposit over for our next pageant.
Deadline for deposit and entry form is July 22, 2003.

Judges score sheets will be available after the pageant. Late Entries will inquire a $15.00 late fee. We will have to enforce this fee. Please get your deposits in on time! It is very hard to order awards and gifts for late entries. Please try to get those in on time. Please be on time the day of the pageant. If you are late then you will not compete and no refunds will be given. If there is a problem please let the directors know!

I WILL NOT HOLD DAZZLING DOLLS OF KENTUCKY PAGEANT PRODUCTIONS INC., JUDY MONHOLLEN, JESSICA MONHOLLEN, JULIE LAMBDIN, WORKERS, JUDGES OR THE HAMPTON INN of Corbin, KY RESPONSISBLE FOR ANY STOLEN/LOST ITEMS, ACCIDENTS, INJURIES, OR ILLNESS THAT OCCURS BEFORE, DURING, OR AFTER THE PAGEANT. I DO UNDERSTAND THAT NO REFUNDS WILL BE GIVEN AND THAT THE AWARD/AWARDS THAT WILL BE GIVEN CAN BE REDUCED IF FEWER THAN 15 CONTESTANTS. I DO UNDERSTAND THAT DIRECTORS HAVE THE RIGHT TO SUBSTITUTE AWARDS.
I ALSO AGREE THAT PHOTOS TAKEN BEFORE, DURING OR AFTER THE PAGEANT CAN BE USED FOR PUBLICITY FOR DAZZLING DOLLS OF KENTUCKY, AND MAY BE PUBLISHED ON THEIR WEBSITE. BY SIGNING THIS FORM ALL RIGHTS AND FILE CLAIMS WILL BE WAIVED. We reserve the right to lower awards if not at least 15 contestants. We reserve the right to combine age groups if not 3 or more in a category.
