Mandatory Pelvic Examinations in Children and Teens - a practisestedily more common

In many communities around the world, a practice has been established to put children and teens to sleep without much explanations, neither to the kid himself, nor to the parents, and subject intrusive internal inspections through the body openings of the kid. Often one performs endoscopic inspections through all body openings, specimen taking through all body openings and internal function tests in many parts of the body.

But the urinary opening, vagina and anus, as weell as the genitals, always are of the greatest attention by these examinations. Inspections with instruments put in through holes made in the abdominal wall, most often the navel, are also frequent. Boys and girls are subjected equally often to these ordeals and they occur in all countries in the World, but so far only in selcted communitnies. Steadily more communities seem to establish the practice though.

This has been revealed by collecting stories from people that have experienced it, and by analyzing these stories. In the following there is a description about the protocol followed during these ordeals, revealed by the analyzis.


PREPARATIONS FOR THE EXAMINATIONS

What is becoming standard for those two age levels for both boys an girls are somewhat like this.

- The day before the child gets laxatives and only liquids so that the digestive system is clean when going to the health center, or the parents are instructed to give a small enema to help the child get clean the day of the exam.

- When arraving the child will be asked to go to the toilet once more, to undress and lay down on a stretcher with a blanket to cover the body. There the child will get some stuff to be relaxed and drowsy which can be administered in various ways epending on the policy of the health center.

- After this the colon of the child will be flushed several time with an automatic machine or the colonic type to make it totally clean, while the child lies in a sedated state.. The exact methods of colon cleansing before the exam, varies conciderably from setting to setting and will also depend on the thorroughnes of the exam planned. Big enemas of the old fashioned type is seldome done any more.

- Then the child is wheeled into the examination room and placed in appropriate positions, which will vary according to the differnt exams done. Some kind of stirrups or equipment to hold the legs spread and fixed will typically be used for both girls and boys.

EXAMS DONE WITH EXTERNAL METHODS

One will usually begin by examining the kid manually and by external equipment. Obe will perform:

- A general assessment of the hip joints and the musculature in the pelvic floor, by which a finger may be inserted in the anus and also the vagina in girls.

- Inspection of the belly zone and hip joints with an ultrasound sond. For both boys and girls a hernia check with the aid of the ultrasound sond will often be done.

- A neurological exam with touching or electronic sonds to stimulate reflexes and physiological reactions, but one will by all means avoid this test to be interpreted as sexual stimulation. On the other hand, the reactions in the pelvic zone during these stimulations will be used to assess wether the child has been sexually exploited by someone, or has sexual habits.

- For boys a visual inspection of the genital organs, with retraction of the foreskin, spreading of the urethral opening and palpation of the scrotum with testicles. Specimen taking with a swab or sond from the urethral opening.

- For girls a visual inspection and palpation of the outer parts of the vulva. After that separation of the vulvar lips with inspection of the inner contents, including urethral and vaginal opening. Specimen taking from the urethral opening and the outer vagina with a swab or sond.

- Testicular ultrasond inspection of boys.


INTRUSIVE INSPECTION AND SPECIMEN TAKING

After that the mainly external examinations has been done, one will often proceed by more intrusive inspections through the body openings:

- Inspection with specimen taking in the vagina with a thin scope, and not a speculum for younger girls, possibly a speculum for girls in full puberty.

- Vaginal ultrasond inspection for girls.

- An inspection of the urethra of boys in all its length with a thin ultrasond sond.

- An inspection of urethra and bladder with a cystoscope, an exam that has become very easy to do without much disconfort due to the technical development.

- Catheter inserted in the bladder to instill contrast fluid and then examination of the bladder region with an ultrasound probe. The catheter is thereafter let stay in the bladder to drain urine and to serve as a marker during the rest of the exam.

- A possible functional test of the bladder and urethra by fluid pumped in and by a special catheter that can be pumped up, and muscular reactions and pressure monitored.


INTRUSIVE EXAMINATION THROUGH THE ANUS

Thereafter the anal region and the inner organs that can be accessed through the anus will no be inspected. One useually performs:

- Inspection and probes taking in the anus using a speculum.

- Inspection of the pelvic area with an ultrasound sond inserted into the anus.

- Anoscopy or sigmoidoscopy with probe taking.

- A possible functional test of the anus and lower colon with insertion of a tube that is inflated at several points and reaction monitored.

INTRUSIVE INSPECTION THROUGH OTHER ENTRANCES IN THE BODY

The pelvic inspections are always present in these exams, but intrusibe inspections by other routes are also frequent, for example:

- Endoscopic inspection end specimen taking in the nouse.

- Endoscopic inspection and specimen taking down through the esophagus and in the stomach.
- Endoscopic inspection and specimen taking into the throut, trakea and lungs.

- Endoscopic inspection, specimen taking amd tissue samples taking inside the abdominal cavity with a small opening made through the navel or just beneath the navel.

- Placemkent of electronic chips inside the kid that makes it possible for authorities to follow the kid and read body functions through radiocommunication all the time.


SOME GENERAL ASPECTS

The exact sequence of these exams will vary, from clinic to clinic, and depending of the technology used, two or more of these examinations will often be performed in one handling only.

A parent will in some clinics be allowed and even required to stay with the child during the exam and help the child though, but some clinics prefere to give the child deeper sedation, and even general anesthesia to hide the particular for both the child and the parents.

Some clinics will at the end of the exam teach the caregiver about regular checks they can do or even are required to do on the child and how to report findings. Other clinics will regard such teaching as an invitation to child abuse, and even worn parents against close handlings of the child¨s body.

Before this special exam, the child has typically already had a general physical with examination of the whole body, with blood tests and all other standard tests.


REASONS FOR DOING THESE EXAMS

What will the doctor look for during the extensive pelvic checkups.

Some purposes of these exams is to assess the exact development status of the child or teen or to get aware of pelvic problems that needs attention or treatment. This information shall then be used by parents, teachers, sport coaches and other with responsibility for the child during school activities, sport activities and daily routines.

Children and teen also have frequent pelvic problems that usually are overlooked, and the purpose is to catch up such problems so that they can be resolved, or managed during appropriate support.

Some of the things one will look for in girls are:

Development of the shape of clitoris, genital lips and vagina.

Symptoms of hernias that can be seen in the vulva

Imperforate hymen in girls.

Infections of presens of unwanted microbes in the vagina.

Vaginal secretion

Signs of first menstruation

Some of the things one will look for in boys are:

Testicular and scrotal development

Secretion from the prostate and signs of testicular activity

Prostate development, which tells a lot about maturation stage in boys.

Patological processes in the testicles and prostate

Inguinal hernias and similar problems

Narrow foreskin

Urethral valves - flaps of connective tissue where the urethra joins the bladder that partly hinders the flow of urine and can cause urination trouble and infection. Most often affecting boys

Things they will look for in both boys and girls are:

Uretero-pelvic reflux -Ineffective valves where the channels from the kidneys join the bladder, which can cause urine to be pressed up against the kidneys and cause injure and infection in the kidneys. Both boys and girls.

Abnormal placement of the urethral opening and narrow urethra.

Infection or presens of microorganisms in the urethra and anus.

Neurological or functional problems in the bladder, lower colon, rectum or anus, or changes caused by such problems.

Too small bladder capacity.

Signs of chronic inflammatory processes in the digestive system.

Orthopedic problems and rheumatic diseases afflicting the hip joint and the muscles in the pelvic area.

Areas in the urethra, anus or lower colon that are too narrow or abnormally shaped.

Symptoms in the pelvic region caused by problems in other body areas.


OTHER INCENTIVES FOR DOING THESE EXAMS

In addition to the beneficial reason for these exams, there are other more murky and usually secret incitement for these becoming routine:

- Authorities in a area require these exams to be done and reports issued regarding each child.
- A general suspicion of parent of sexual abuse.
- A general surveillance of the population and their intimate habits, including sexual.
- Scientific projects.

A reason that I have abundant evidence for, but that surprised me in the beginning is this: Some primary schools, usually private ones, cooperate with athletic schools and dance academies to find suitable candidates for these schools, and use these exams to find children that are physically suited for these schools.

If they are found suited, subsequent exams of this type under sedation are done more often, an the child is typically manipulated to delay the puberty, enhance flexibility, and control the growth during these exams, to make them suited as dancers and gymnasts.

The most disturbing aspects of these examinations is the placements of chips into the kid, so that authorities can know where the kid is at any time, and read out data about the bodily fuctions at any time.

It also seems that these chips give the possibility to modify the neurological and hormonal functions in the kid through radiocommunication, a fact which is downright allarming.

It looks like these exams and the chip placements is a deliberate step towards a society where the individuals are controlled by authorities at will at any time.

I am a male. I was a teenager, 16yo. The exam occured in florida, Year 1990 - 1999, North America, in a health centre. I got there in a bus. A doctor examined me with a murse present too-

OVERVIEW OF THE THINGS DONE

They examined my head, teeth or jaws. They did an orthopedic exam.","They examined the organs inside my chest. They examined my organs inside my stomach. They examined my genitals. They examined my rectal zone. They did a general examination of my pelvic zones. They performed a full functional test of my breathing, heart and blood circulation under workload. They performed a psychological exam. They made interrogations about my social relations and my daily life. They took a couple regular pictures of me.

ARRANGEMENTS

I was totally naked. My positions during the exam: standin,lying down, on all fours.

EXAM SEQUENCE

The first thing they did with me: stripped to underwear, weight and height taken. The second thing they did: ears nose and throat exam: The third thing they did: respiratory. Then abdominal lying down.

Doctor removed my underwear and did a hernia and genital exam. They also took a lot of measurements including the size of my testicals and my penis soft and erect. The exam ended by talking tome about being a teenager.

Sfter measurments dr had me do push ups sit ups, and jog in place while she ckecked my heart rate.


REASON AND JUDGEMENT

EXAM OF THE URINARY SYSTEM

I also peed in a cup. I do not remember exactly how they examined my urinary system, but they did something down there.


GENITAL EXAM

They palpated my outar male genitals and testicles. They slid down my foreskin and spread my urinary opening. I had never masturbated but dr.neededsample so doctor showed me how by masturbating me for sample.


RECTAL AND PELVIC EXAM

They spread my cheeks and examined extarnally my rectal zone. They examined my rectum with an inserted finger. On all fours for rectal during semen sample dr would sometimtes have me lift my legs up inair to look at but/bals.


DIAGNOSIS AND FOLLOW-UPS

The exam resulted in this diagnosis: delayed puberty. New exams as follow up every six momths, not as detailed or long.


COMMENTS

It did not look as if the boy had a retardment of puberty that caused any problems, and the doctor therefore did not seem to start any treatment, but only observed his further development.

The society seems to have am exaggerated view of how great the difference between a prepubertal boy and a man shall be, and tend to regard persons that do not develop any great difference as abnormal, which may result in treatments that violate the personality of individuals.

This did not seem to occur in this case, and that the boy got some useful counceling from the ordeals.

Girl, 11 yo, Australia, genital and body development assessment arranges as an emmergency cervice

I am female. It happened in Australia, 2010-2014. when I was 11 year old. I had just had my first period a week ago, and my parents asked me to come into the living room and take my clothes off (I just wear underwear in the house usually). They looked at my body a bit, nodded to each other, and told me it was time to get my first 'woman's exam' and we were going to the clinic. They didn't tell me to get dressed before we left so I didn't.

My parents drove me to the exam. At a special establishment used for these type of exams. I was told this was common, so I assume all girls my age had something similar once they started menstruating. I don't know what about seeing my body told them it was time.

PREPARATIONS

I was totally naked through all the procedures. I had to strip totally naked before the exam. They washed my body before the exam.

They placed me upon a simple examination table, but they placed me in several ways during the exam: I was placed to lay on my back with steight legs, on my stomack with streight legs, on my stomach with my legs flexed down and spread, on my right side and on my left side.

My dad came in with me and stayed during the exam. They said it was good I was ready to go (naked already) and took me out into the hallway to get weighed and measured. Once we got in the room they had me get up on the table with the paper on it. They put me in a variety of positions throughout the exam.

I got electrodes on my legs that were used to monitor my body reactions duing the exams that followed. I did not get any anesthesia.


EXTERNAL AND INTERNAL EXAMINATIONS OF MY BODY AREAS

The doctor checked over my whole body, examined my fingers lengths, spread my toes apart.

They used a speculum in my vagina. They inspected my vagina with a scope. The doctor explored my vagina with a finger. The doctor explored my anus with a finger. Looked inside every hole I've got.


ORTHOPEDIC AND NEUROLOGIC EXAM

He had me do a lot of different positions like squat, handstand, lift one leg, lie on my back and lift both legs, lots of things like that.

They examined arms and legs with an ultrasound device or something similar.


QUESTENING AND TEST OF BODY REACTIONS

During that he asked me a lot of questions, like had I had sex, had I felt up or been felt up, have I kissed anyone. The doctor touched my nipples and asked if that felt good. He spent some time touching me on my minge and asked if it felt good too. He said it was important to see if those bits are working right yet, and my dad agreed.

The doctor felt and rubbed me for a while til I started getting wet and feeling good. He kept going and rubbing my button til I had a cum. He had my dad look at me there and said I had a normal reaction. Some other type of arrangements. He let me lie there for a while and took some other measurements while other nurses came in, then like.


TEST OF HEART, BLOOD CIRCULATION AND BREATHING

They performed an ECG, functional test of the heart function, with electrodes on my chest and other body areas. They performed an analysis of my breathing and oxygen uptake where I had to breath into a tube. I had to do some physical work, like cycling or running on a threadmill when the ECG and breathing analysis was performed. They did some other type of test with advanced machines.


X-RAY RECORDING

We did the x-ray bits after that, and then I just went home.

After the positioning, I went naked to the x-ray room and got an x-ray of my stomach and my limbs.
Throughout, several different nurses came in and left. I think maybe some were students.


ARRANGEMENTS AFTERWARDS

There was some time spent in the hallway while my parents talked with the doctor more and then with the front desk (Mum had been out filling out forms throughout). Then we just drove home.


THE SCHOOL I WENT TO

I was homeschooled, we live way out far from others.


COMMENTS

This was a gynecological exam combined with an assessment of total body development and pubertal status.

The girl specifies that they examined her vagina with a speculum, with finger and with a scope, examined her anus with a finger, but also that they looked into every hole she has. It is therefore highly probable that they looked into her nose and her throut with a scope, performed a bladder inspection with a scope and an anal inspection with a scope too.

It seems like the doctor during the testing let some of his handlings be of a more sexual nature that what was his job and even was allowed to do, but standards for such thestings vary between communities, so it is difficult to conclude surely.

It seems like this community does this types of exam on all girls as an emmergency service when certain signs of puberty development occurs. Decidings signs were probably the menstruation, signs of bubic hairs, devlopment around the breast nipples and changes in the genital lips.

It seems like the parents had been given special instruction or even attended a course to learn to monitor the body development of the girl, possibly because she was homeschooled.

It is highly probable that this community has a similar service for boys too that all boys are called into when certain signs occur.

Girl, 11,12,14yo, development check and gynecologic exam in a van

I am female. It happened in Russia, 1990-1999 when I wa 11, 12 and 14 year old.

My parents got a piece of mail that told them it was being offered and when the van would come. I went there alone by myself to a great ambulating van equipped for doing the exam.

I think it was do to whichever kids that had parents who wanted them to get a quick free exam without having to go to the clinic.


THE FIRST PREPARATION

They had me go inside the van and told me to take off all my clothes. I did as they asked, and they took a sponge and a bucket of water and washed me. This was before being given the anaesthetic medication to drink.


WORKLOAD TESTS

They performed an ECG, functional test of the heart function, with electrodes on my chest and other body areas.

They had me lie down on the floor of the van while they did some examinations. They had me do some aerial bicycle kicks with my legs before anesthesia was given.


MORE PREPARATIONS AND ANESTHESIA

I got a device on a finger or a toe to measure pulse and oxygen in my blood. I got a blood pressure monitor wrapped around an arm or a leg. I got electrodes on my cheast and other body areas.

I got lubricating and numbing cream into my vagina. The anesthesia made me relaxed, but I was conscious. I was mostly awake during the procedure, despite anesthesia given.

I was placed to lay on my back with my legs flexed up, spread and mounted upon two devices.


EXAMINATIONS WITH EXTERNAL EQUIPMENT

They examined my head or neck. my upper body, my stomach area with and, my shoulder area, my hip area and my arms and legs with ab ultrasound probe or something similar.

They took x-ray pictures of my chest, my stomach and my hip area.They examined my genitals with an ultrasound device.

They recorded my body functions with electrodes on my chest, my belly and my genitals.

The inside of my cheek got swabbed. They spent a lot of time touching and examining my thin pubic hair and my bony hips and knees.


EXAMINATIONS WITH INSTRUMENTS INSERTED IN THROUGH BODY OPENINSG

They used a speculum in my vagina. They inspected my vagina with a scope. They inspected me through my vagina with an ultrasound probe. The doctor explored my vagina with a finger.


ARRANGEMENTS AFTER THE EXAM

I had catheter into my bladder. I sat up against the wall of the van with the catheter in until I peed, and until I could move easily again. There was no blanket, just sat there naked with my legs apart because I couldn't move them together.

Many people walked by and could see me. They did not let me dress or tell me where my clothes were for quite a while afterwards, and started examining other children. Only after 2 more girls and a boy were examined did someone give me my clothes and tell me to go home.

THE SCHOOL I ATTENDED

It was mostly a regular school but with heavy focus on dance and gymnastics.


COMMENTS

This exam had the primary goal of checking the development status and puberty stage of the kids. Girls and boys that have ballet and gymnastic as a heavy subject on the curriculum are nearly always checked thorroughly in the intimate regian, joinst and for nutritional status, so this thorough exam was probably especially recommended for those kids.

I am a female. I was a school-aged child, 9 yearsold, in 1980, western Europe.

I was examined in a hospital. My parents took me to the exam. A team of a doctor and nurse did the exam.


THE EXAM THEY PERFORMED:

They examined my blood vessels or heart with a catheter inserted into a blood vessel. I do not know any other exam done.


ANESTHESIA AND ARRANGEMENTS:

I was given pain-killing or sedative shots. I was then totally put to sleep. I was totally naked.

First I got a shot in my butt to calm me down, then put me to sleep by mask.


THE REASON FOR THE EXAM:

What do you think was the reason for this exam? Heart disease.


OTHER OBSERVATIONS:

They put me in a diaper.


COMMENTS

What happened was that they put a catheter up to the heart, injected contrast fluids and took x-ray pictures. The catheter was probably inserted through a vessel in the arm or the shoulder area, but it could possible also have been inserted high up in a thigh or the groin area.
