Some do, yes, but many are just 'theme with slight variation' pieces that start to blend together after a while, and are so badly written that you want to gouge your eyes out with a spork. This is why Bleeprin was invented.

Within the boundaries of what the author of the original set, yes. And even with AUs or 'what if's, there are still boundaries you need to stay within. Too many do not do that. To make a fic work, you need to know what you're writing about before you write about it, which usually requires (brace yourself now!) research. Yes, it takes time and it can be boring, but it's incredibly annoying to read a story and see that the writer has merely slapped modern 21st-century American values and morals and speech and all that over a fictional background. Doesn't work that way. I'm going to be writing a fanfic about Elfhelm, one of the Riders of the Mark, and that's going to require me learning about Rohan and its people. If you really enjoy an area, then the resarch isn't a drag, it's very enjoyable.

Yes, some people are, and some people can. But many of those who think they can do it well cannot by any stretch of the imagination, and suceed only in making you spew "technicolor vomit" and making your royally PO'd because you used rape/abuse as a plot device without bothering to give it the respect it deserves, and you further mangled it by having the victim recover almost immediately because they have the love of a good man *coughLegolascough*.
