Your coolant temperature sensor (black box) is broken. It is located right next to the main relay (brown box) under the dash on the driver's side. If you've replace the main relay before, then replacing the coolant temperature sensor should be easy. If not, then a brief instruction on how to replace this sensor is located further down the webpage.
