I use Photoshop.

If you have a BIG source file (over 150dpi), and the background colour is pretty different from the subject, you can use the magic wand tool to try and mask the background out, then delete it. I usually set the tool between 15 and 25, anti-aliased, and set to continous. After I'm done I sometimes go in with the angular lasso tool to fix any breaks in the outline.

if the background's too similar/dark etc, I just use the lasso tool to catch everything manually. Lots of clicking! :-)
