And, I can explain how those events in question happened.
You were questioning:

1. The transfer of Smith into the "real" world.

Well, if you recall from the Matrix reloaded, Morpheus asked Neo what Smith was doing to him...and he said that he didn't know but it FELT LIKE DYING.

So, how did he get into that body in the first place right?
Well, soon after the SEX scene... there were two guyz from the real world running away from something...
They had a disk from the oracle, and they had to bring it back safely... one guy escaped with the disk and the other
guy got caught by Agent Smith and clonned himself. From there he took the phone and transfer his mind to that body.

So how is it possible to take over the mind in the real world?
Well, how is it possible to take over the mind in the virtual world? I mean Agent Smith was able to take over that person's body and mind in the matrix....where Neo explained it as dying. So how is that possible cause that guy's mind who Smith invaded was unplugged. Remember, in the first movie, they said that anyone thats hard wired to the system is a potential agent. ONLY ones that were hard-wired to the system.

Ok, I think Im raising more questions than answering them...
So what I think happened is that when Agent Smith took over that body and mind.... and if it was like dying....then that dude must have FLAT-lined, but he didn't, instead it was a definite body-mind take over....so he was able to pick up the phone and transfer Agent Smith's mind into that body.
