The most detailed coverage for what you want is probably found in Richard Sair's "The Book of Torture and Execution" (Toronto: Golden Books, 1944). For whatever reason--well, actually, because Sair's name occurs nowhere on the book--it is generally catalogued under Arnold Hirsch, M.D., who edited it. You can also find very thorough information in "Le Musee des Supplices" (I can't remember the author), the very bizarre "Erozismus im Hexenprozess" (I can't remember the author), and the article "Tormento" in "Larousse Enciclopedia Gran Universal Ilustrada" (older editions, pre-1940, particularly).
