You should check with your Department of Children and Family Services with regard to getting legal custody of the child. I am not sure if they can help you, but it is worth a shot. Explain to them the trouble you had getting the child the surgery she needs, and how that could endanger the child if medical attention was urgent and you couldn't get them to use your authorization.

If they can't help you, you might be able to find the forms that you need at http://www.findlaw.com and only have to pay the filing fees.
