I'd say you haven't gotten over the crash. Even though you think you are ok it sounds like you may subconsiously feel bad that you got out alive and the person next to you didn't. Maybe talk to someone about how the crash made you feel despite however long ago it was. As for the ghost - hes probably there because you won't let him go. You think he shouldn't have died. And hes probably trying to let you know hes gone to a better place and its ok. I think that when you realise that you can't change whats happened you can let go and he can go too, to wherever his new life takes him.
