Please recall that Simon saved the life of an unknown patient in the hospital just a few minutes before Jayne, River and he were captured by the guards. In doing so, Simon risked his sister, himself and his mission (possibly even the entire Serenity crew). Is it reasonable that the same man who did this would not care whether the guard lived or died?

Actually, I'd say yes. Simon took a risk in stopping to save the life of a stranger, it's true. But at that point he was free and had no reason to believe the plan was compromised, or that stopping would significantly endanger the mission. In the Brawl in the Hall, they were already captured and he was facing the certainty of losing River to the Blue Hands again if he didn't fight to get free. I submit that Simon was scared and desperate, his priority was escape for River and himself, and whatever happened to the guard was at best a distant second to that overriding consideration. I don't think Simon meant to kill the guard, I just don't think that Simon didn't mean to not kill the guard either - but though I think Simon has a significant capacity for ruthlessness in defense of River, I do acknowledge that there is room for reasonable people to disagree on this.

What I don't think is reasonable is the idea that any amount of medical training would allow someone to minutely gauge the amount of pressure he's applying via his knees to the neck of a struggling opponent. It would be hard enough if his hands were free - but to do it with his knees?
