Well, if the next closest ortho that is covered by your insurance is an hour away....wow, that's tough. You could go for a consult or two "uncovered" and pay out of pocket if there is a charge (some orthos consult for free). Just to ease your mind.

Headgear is a difficult subject for adults. There have been several adults who read this site who are in (or have been in) headgear. Basically, the more you wear it the quicker it works. So if your ortho is telling you to wear it 18 hours a day and you only wear it at night, you'll wind up wearing it months and months longer, and it may not do the job it is meant to do in the proper timespan. I know that stinks, but that's the way it is.

You are correct that one ortho told me I'd need headgear for six months. I had a newborn baby to care for, and headgear was out of the question at that time in my life! But he was the only one who had mentioned that to me. My current ortho doesn't like to use headgear on adults, mainly because he knows that adults don't want to wear it. Sometimes, however, it may be the only way to achieve specific results.

I don't know what you do for a living, but aside from the kneejerk aversion to headgear (which most people have), would it actually impact your life and your career negatively to wear it in public? Or is it just that you'd feel embarrassed by it? If it is the latter, I can tell you that the embarrassment will wear off. Most of the readers on here who have worn headgear wound up not caring one iota that they were in pubic with it, after a while.

Anyway, if it is a major issue for you, you'd better have a frank discussion with your ortho and see if there are any other methods he can use that do not involve headgear.

Good luck!
