Hi,

i know exactly how you feel. I also had major worries about having teeth extracted. I also had 4 premolars extracted. On the top left it was the second premolar and on the top right first. And i can tell you now, having a first premolar extracted does mean that you'll be walking around with a fairly visible gaps (when you smile, i'm also 22 by the way) for a couple of months at least. It's been four months for me now and my gaps are much smaller and don't really bother me that much any more. (although i'd love for them to be fully closed now)

In your case it seems like you have some crowding. Extractions are usually done if you have either crowding of the teeth or if your lips (profile) is protruded or both. My case was both. With the extraction of premolars the protrusion of the lips can (but doesn't necessarily have to) be reduced. You seem to fear what extracting teeth might do to your smile. I had the same worry but found numerous articles saying that extractions (if done for the right reasons) have a positive impact on both profile and smile. The one research article had 50 people in pictures smiling, some with and some without extractions and asked both lay people and orthodontists to judge whether they though their treatment involved extractions. Both lay people and orthodontists had a 50% success rate. Basically a toss of a coin could would have been just as accurate, meaning that it seems that after treatment it is impossible (v difficult) to tell whether extractions were involved or not. I found other articles stating that the archwidth does not decrease with extractions neither does the no of visible teeth. (one of your worries)What i think happens is that one of you molars then looks like it is a pre molar.

I think you will find it impossible to find one credible article that says that extractions cause a poor smile. As to molar extractions, definitely an option, eventhough the gaps take longer to close. but if you don't feel like you have protrusive lips and want you profile to remain as is, then this could be an option (eventhough i hear molar extraction treatment is less stable and much less common than premolar extractions)

The ortho has to create space somehow. Ask what other options there are eg an expander?
