Subject: archwire, missing bracket, molar band

For a about four months now I've had a missing bracket(the Dr. opted not to replace) On the tooth right before the molar band.Could this be a problem? The wire has popped out of the bottom left molar band. The molar band seems a little loose. Has anyone had brackets left off of certain teeth? Does this seem normal? My bottom teeth are still very crooked.
