Ah so watched the sixty minutes item with the interview of the top level electronics guy who was on the rig and just barely managed to survive, Very enlightening.

Problems with the rig and it's equipment, problems with the blowout preventer. an unnamed person in the crew who apparently accidentaly hit something while a pressure test was in progress that caused the drill to rotate ( or that was what was implied ) and damaged the seal used for pressure testing , rig management ignoring a report that there were chunks of the rubber seal coming up out of the well when mud recovery was taking place, the BP man on board pressing for a faster schedule after the intital well hole drilled was damaged and needed to be redrilled elsewhere after it was sealed becuase BP was pressing for a faster schedule at that time when the type of strata they were drilling through made normal speed slow it down to six weeks when they should have been able to finish inless than a month had they not encountered that type of strata, and that caused the well hole to be fractured and loss of cementing compound used to seal that well hole during drilling, restarting with the well that blew making more of a pressure situation with BP pushing for a faster yet schedule. The BP guy overrulking the proceedure for cementing plugs used to temporarilly reseal the well casing which later failed due to incorrect readings from the pressure tests made with the damaged seal, one of the redundant control units at the BOP not properly functioning leaving only one to work or fail and no backup......

Well you get the idea, same as usual in the oil biz with the owners interfering with the professionals actually doing the drilling and establishment of a well.
My buddy who did an entire well field in Kentucky for a lexington outfit that created a number of low production wells thatt would probably have been productive for decades had a good contract that kept him fully in charge of the operations or the owners would have done forced recovery of the oil to take advantage of the market while he was still establishing the well field. After he left the company engineers from the home office did exactly what he told them they should not do and ruined the wells in about a year. Oly loss was to the company who might still have had those wells producing. Maybe 20 to 30 percent of what was there was removed while the balance is still in the ground awaiting someone who can figure out how to get the rest out with better technology.

Yep sounds like greed at the top or very close to it again was responsible for the gulf spill disaster and whatever damage it will continue to cause.

Had all the safety rules and proceedures been followed and in place most probably the well would not have had any problems at all.

Now in addition we find that the fed agency responsible for enforcing the rules let BP slide on ionspections and compliance for years and that there may be rigs out there with the potential to do even more damage than this one if something goes wrong thanks to bad practice, poor construction, or operator error based on the incorrect info without the inspections and enforcement that should have bee part of the entire operation.

Would not trust BP as far as I could throw their entire management wrapped up in their entire mass of existing infrastructure.....
